We are committed to protecting your privacy and follow HIPAA guidelines to safeguard your personal health information. All records and conversations are kept confidential and will only be shared with your written consent or as required by law.
Cancellation Policy
We understand that life happens. If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice. Missed appointments or late cancellations may be subject to a $50 cancellation fee.
We operate primarily as a self-pay practice. Payment is due at the time of service.
We accept major credit/debit cards, HSA/FSA, and checks.
We do not bill insurance directly, but can provide a superbill for you to submit to your insurance company for possible reimbursement.
Package or membership fees must be paid in full at the time of enrollment unless otherwise arranged.
